out and return the following forms to her Team Rep:
Commitment Form
Medical Release Form
Player Information Form
Copy of Birth Certificate
Also, do not forget to review the player expectations page.
To fill out the forms on your computer cut and paste them into your word processor or spreadsheet program and then print and sign.
